Double outlet left ventricle is one of the rarest and least well understood congenital anomalies the heart, with means that more than 1 l/2great arteries arise above the morphologically left ventricle. We experienced a case of double outlet left ventricle (S,L,L) with subaortic ventricular septal defect and pulmonary atresia in a male neonate, which was diagnosed at autopsy. A 4 day old male patient was admitted to our hospital due to jaundice, poor feeding and abdominal distention. His clinical features and peripheral blood smear revealed congenital myeloblastic leuke- mia which confirmed by autopsy. Chromosome study showed 47, XY, +21 in Karyotype.
